PAD First office visit
Newborn Visit: Usually takes place within the first two weeks of the hospital discharge. You will need to bring the insurance card under which your child is or will be covered. Hospital discharge papers, and immunization card, drivers license of parent or guardian.
If you are transferring from another medical office you must bring the following:
- Drivers License
- Immunization record
- Insurance card: Verify with your insurance company that your child is covered under the insurance that your present to us
